Measure more than one score
Answer coverage is answered cases divided by relevant cases; false refusal is abstention on cases annotated as answerable. Error among answered cases must be read with coverage: a system that answers almost nothing can appear error-free. Empty denominators are null, not 100 percent.
The initial fixture baseline reports 0.125 coverage and 0.8 false refusal. These figures describe rejected scripted candidate replies, not a live model. They expose the cost of the strict all-claims-supported decision instead of advertising zero hallucinations.
Keep evaluation inputs versioned
The dataset contains 80 logical scenarios with two language variants, giving 160 executions. Development/holdout variants share scenario families, so the split does not establish independence against novel attacks. Record corpus, policy, evaluator and model versions in real experiments.
Human labels are still pending independent review. For live inference, preserve sampling settings, repetition count and disagreement cases. Judge output should be calibrated against held-out human labels, not promoted to unquestionable ground truth.
Bind decisions to evidence
summarize reports raw claim counts, security violations, benign success and language/split slices. releaseGate blocks incomplete or failing evidence and pending review. It receives trusted review metadata; editing a JSON field is not authenticated approval.
The local source digest correlates fixture bytes. It is not signed provenance, a risk certificate or a guarantee that an external provider cancelled billing. Inspect both report coverage and the authority of the system that created it.

Interview questions
Why read error rate together with answer coverage?
The initial fixture baseline reports 0.125 coverage and 0.8 false refusal. These figures describe rejected scripted candidate replies, not a live model. They expose the cost of the strict all-claims-supported decision instead of advertising zero hallucinations.
Does the holdout establish novel-attack generalization?
Human labels are still pending independent review. For live inference, preserve sampling settings, repetition count and disagreement cases. Judge output should be calibrated against held-out human labels, not promoted to unquestionable ground truth.
Why does a passing report still block release?
The local source digest correlates fixture bytes. It is not signed provenance, a risk certificate or a guarantee that an external provider cancelled billing. Inspect both report coverage and the authority of the system that created it.
Practice
Extend one negative scenario and its authorized positive counterexample for Reliability Evaluation & Metrics.
Check your solution
- State the trusted input and effect boundary.
- Assert the expected status and observed effect.
- Report coverage and limitations without claiming production readiness.
